You must complete FERPA and Statement of Responsibility trainings via myUK Learning before submitting this form.
Approval from your Director of Graduate Studies (DGS) or Director of Graduate Certificates (DGC) is required for the access you are requesting.
Instructions:
Log-in Check: Use this link to verify whether you can currently access Slate.
Complete All Fields: Enter your personal and departmental information, including your LinkBlue ID and @uky.edu email.
Select Populations and Roles: Choose the appropriate programs and access roles required. If needed, you may request access to multiple programs in one submission by clicking the "Add New Population/Role Combination" link below.
Certifications: You must acknowledge the completion of required trainings and confirm approval from your DGS/DGC.
Submit Your Signature: Enter your full legal name to certify the information provided is accurate.
Confirmation: You will be contacted via email once your access has been reviewed and processed.
